Step 1: Getting the information
To get started, a staff member of our team will give you a call to ask for basic information (demographics, services needed, and insurance information) and to determine if Cara Behavioral Health team is right for you. Then, you will receive a link from Alma in your email to add your health insurance informations.
Step 2: Filling out forms and scheduling
Once we receive your information (full name, phone number, e-mail address, and photo of your driver’s license), and we deem that our provider is in-network with your insurance, you will receive a link from Alma to fill out the intake form, consent, and assessment forms.
You will also be scheduled for a 60-90 minute tele-health appointment with our provider. Follow-up appointments are between 20-30 minutes long and you are re-scheduled based on what you and your provider agree upon. By the end of the session, you and your provider can start developing a personalized treatment plan.
Step 3: Complete Form
The intake and consent forms need to be reviewed, completed, and signed prior to your first appointment. If your forms need to be printed, please go to our Alma telehealth page. If your forms are not completed prior to 24 hours before your initial appointment, that appointment will be canceled.
